The Proliance Center for Outpatient Spine and Joint Surgery uses the Simple Admit platform for patient registration to ensure your experience the day of your surgery is smooth and safe. This is a requirement for your surgery or injection and any delay in completing the registration could result in the cancellation of your surgery or injection.
RETURNING PATIENTS: If you are a returning patient and have previously registered using Simple Admit, you are required to update your information each time you have a surgery or an injection. To update your information, please call 877-848-4726 and they will either update your information while on the phone with you or email you a link to access your previous registration, so you do not have to start over again.
NEW PATIENTS (first time at COSJS): Please follow the instructions below to complete the Simple Admit registration for your surgery or injection.
Please use the instructions below to access Simple Admit. Surgery Patients are those having a surgical procedure (including spinal cord stimulator trials or permanent implantations, or other implants) and Injection Patients are those who are having an injection only.
